If you click on the link above you will get a page like this. Scroll down to see the form to be filled.
Step 3:-

Enter the same name as in your passport in the first column. Fill in Date of Birth, Gender, Flight Number, Seat-Number and PNR Number.
If you do not know the seat number, fill in '00' If you do not know the PNR number, do nothing there.

Step 4 :-

In the next column, select your home country, fill in your passport number, and upload a copy of your passport. Passport‌ copy should not exceed 1 mb. In the next column, select the date on which your flight will land in India.

Step 5 :-
In the next column, select the country from which you are coming, and if you are using a domestic flight, select 'yes' or 'No'.
Next upload the vaccination certificate below 1 mb as pdf.
Next, select the place where the final vaccination was taken and the date taken.

Step 6 -

Now you need to upload the rtpcr negative certificate, which is also a pdf of less than 1 mb. rtpcr must have been taken within hours ago.
Then enter the date taken by rtpcr, the city where you boarded the flight, the name of the airport where you landed and the state.

Step 7 :-
After that, fill in the address of the destination. Fill in the house number, village, place, state, district, zip code, mobile number, alternate mobile number and email id.

After filling in how many people are traveling, click on the three points below. Then click on the Submit button.

If you submit, you will get a page like this. Make a note of the reference number in it. Your self declaration form will be sent to your email.
